By clicking the Online Athletic Team Registration above, Parents and Athletes will be digitally completing the following forms required by the Ponaganset Athletic Department.  Registration MUST be complete prior to participation by all athletes, with no exceptions.

• Parent Acknowledgement
• Insurance Verification
• Concussion Information
• Assumption of Risk (2024-2025 SY ALL 6th, 7th & 8th grade/ 9th grade or new to athletics)
• Student's Physical Exam

Students participating in Athletics must upload a copy of their current physical examination to the Athletic Department  before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals must be uploaded prior to the first day of practice/tryouts. Save date, then upload the form.

Physicals are valid for 1 Year from the date of the last exam.  Please contact the athlete's primary care physician for a "Bridge Note" if the next physical is beyond 1 calendar year.

Assumption of Risk form must be uploaded to Step 5 in .jpeg format once notarized
